What child doesn’t want to be rewarded for academic achievement, school attendance and healthy habits?
The Sarah Bush Lincoln Toledo Clinic is encouraging students in grades Kindergarten through 12 to improve academic achievement, school attendance and healthy habits by entering them into a drawing, for a prize each quarter.
Students and patients of the SBL Toledo Clinic may bring in their report card each quarter. For every “A” and “B” grade they will receive a ticket to be placed in a quarterly prize drawing. Students may also receive at ticket for perfect school attendance, preventative health visits or for participation in any type of health or wellness educational activity or group.
To enter the first quarter drawing students must bring in their report card or proof of physical activity by Friday, November 15, the second quarter drawing by Monday, January 20, 2014, the third quarter drawing by Friday, April 4, 2014 and the fourth quarter drawing by Friday, June 20, 2014.
At the end of the school year, all tickets from each quarter will be combined for a grand prize drawing.
Drawings will be held at the clinic and students do not need to be present to win. Winners will be contacted by phone.
